Resident Information
Staff are responsible for maintaining the privacy of residents
and resident information. Every staff member holds a position of trust to maintain
confidentiality.
Information concerning residents is confidential. This includes
anything written, verbal or overheard about the residents and their families and
their care.
Residents' records are confidential documents. This includes
medical and nursing records, all types of care plans, treatment and all other
lists identified by residents names.
For the purposes of the Yallambee policy, information
will be deemed to be that information which is collected about residents, relatives,
contractors, volunteers and employees
The primary purposes for which personal and health information
is collected is -
- To process applications for residency
- To assess the level of care appropriate to the residents
needs
- To make decisions about the level of funding that a resident
would be entitled to receive
- To maintain contact with residents family and friends
- To provide staff and maintain resources to meet "Yallambee's"
vision
Access to information may be requested on the appropriate
form and directed to the Privacy Officer Yallambee Traralgon Village for the Aged Inc. A fee for the provision of information will apply. Access
to information may be withheld until the set fee is paid.
Yallambee will take all possible steps to ensure
that any request for information is not vexatious and will verify the identity
of the person seeking access to information
Disclosure of resident information to any individual or organisation
will be in accordance with the provisions of the National or Victorian Health
Privacy Principles and other relevant legislation and / or guidelines for the
disclosure of personal and health information.
Yallambee formally recognises its obligation to
collect the necessary health and personal information in a lawful, fair and non-intrusive
way.
Students on placement and volunteers will sign a Confidentiality
Agreement with Yallambee which reflects the principles outlined in
this policy.

Information collected for the purpose of providing an aged
care service will be considered health information. This includes
- Personal details
- Personal Information relating to an individuals health
history
- Medical Examination findings
- Health Diagnoses
- Plans of management and actions taken
- Information connected with the donation of body parts

is committed to respecting the privacy of your personal and/or health information
It is bound by a set of National and Health Privacy Principles
(NPPs & HPPs) which establish the benchmark for how this information
should be managed. These Principles have been adopted by Yallambee
as part of its operating procedures.
Personal and health information will be dealt with in
a uniform manner having the highest regard for its security and the prevention
of unauthorised access

Complaints concerning any aspect of the collection, storage
and /or retrieval of your personal and /or health records will be dealt with promptly
and in accordance with the Yallambee Complaints Policy.
